I know these things tend to break the rear leaf spring perch when they get old, but that wouldn’t explain the bed being all jacked up.

That’s one of the Heavy Bed-Front Payload packages, so you can carry something heavy in the front of the bed near the cab, and then the truck looks normal. It also has an advantage for off-roading
as the frame
curve improves the break
over angle significantly
, though the departure angle suffers a bit.
